Privacy Notice

THE PURPOSE OF THIS NOTICE

This Notice has been created to help you understand what kind of information we collect from you and how we process and use it. The information we collect from you is called personal data.

 

WHAT IS PERSONAL DATA?    

Personal data is such things as your name, age, address, date of birth, gender and contact details.

 

Personal data may also contain special categories of personal data such as your health, racial or ethnic origin, political opinions, religious or philosophical beliefs, trade union membership, genetic and biometric data, or data relating to sexual orientation. 

 

PERSONAL DATA WE COLLECT

So we can provide you with our therapy services for you, we will collect and process personal data about you. This data may be provided by you when completing our online contact form, when you phone us, when you email us or when you complete our client history forms.

 

We will collect the following data from you:

 

  • Personal data such as your name, address, date of birth, gender, and contact details.  

  • Special categories of personal data such as health and lifestyle details.

 

If you object to the collection and use of your personal data we may be unable to provide you with our services.

 

WHY DO WE NEED YOUR PERSONAL DATA?

We need your personal data so that we can find out about you and what your therapy needs are. In order to provide the most effective treatment for you, we need to know this information. We also need to understand your medical history in order to provide you with safe and effective support and to know that there are no contra-indications to the therapies being provided.

 

By using our services you should understand that you are forming a contract with us and we need to hold a record of that contract. 

 

DATA RETENTION

Where you have contacted us by website contact form, direct email or phone, but do not wish to proceed with any of our services, we will not hold any of your personal data beyond that first initial contact with you. Any personal data that you have provided us with at that point will be confidentially destroyed. 

 

Where you have contracted with us for our therapy services, we will hold your data for a period of seven years from the date of your last appointment with us. At the end of this period your data will be confidentially destroyed without prior contact to advise you of this. 

YOUR RIGHTS

You have certain legal rights regarding the use of your personal data. These rights are given under the Data Protection Act 2018 as:  

 

  • The right to be informed about the personal data being processed;

  • The right of access to your personal data;

  • The right to object to the processing of your personal data;

  • The right to restrict the processing of your personal data;

  • The right to rectification of your personal data;

  • The right to erasure of your personal data; 

  • The right to data portability (to receive an electronic copy of your personal data);

  • Rights relating to automated decision making including profiling.

 

You are entitled to exercise your rights at all times. 

 

PROTECTING YOUR DATA

We will take all steps to protect your data at all times. Your personal data will be held by us securely and will not be made available to third parties unless requested. We will not pass on your data to third parties for marketing purposes. 

 

If you wish to have access to your records at any time, then you simply need to put that request in writing to us. We will also accept third party requests for your information if such requests are made as part of an ongoing medical or legal claim that you are bringing. Such requests will only be granted if made in writing. Under no other circumstances will we share your personal data with any other third party.

 

COMPLAINTS

If you are unhappy about the way in which we process your personal data please contact us.  You also have the right to complain to the UK’s data protection supervisory authority, the Information Commissioner’s Office (ICO).
